What to Bring:
1. Completed Application: Form DS-11: Application for a U.S. Passport (The Library provides blank applications on-site, and you may also print the form from the US Department of State website).
Please see the Department of State website for forms for other situations, such as lost passports or legal name changes.
2. Proof of US Citizenship: bring at least one of the documents below; it must be an original, not a copy:
3. Passport Photo: Provide a photo fitting the following criteria:
Please note that the Library does not provide photo services. We encourage patrons to choose or purchase a new passport photo before making their appointment.
Applications typically require two payments: One to the US Department of State, and one to the Library.
The fee for passports is set by the State Department and can be found online along with a list of frequently asked questions about payment. This payment must be in the form of a check or money order only; if you pay by check, it should be made payable to the US Department of State.
The Library charges a $35 non-refundable execution fee per application. This payment may be in the form of cash, a check, a cashier’s check, or a money order; if you pay by check, it should be made payable to the City of North Miami Beach.
If you are unsure what your fees will be, the Department of State maintains a fee calculator that will estimate your costs for you.
Please note that routine processing typically takes from 8-11 weeks, while expedited processing typically takes from 5-7 weeks. If you would like to check on the status of your application, or have urgent travel plans and need to travel within 14 days or less, please call the National Passport Center (NPIC) at 1‑877‑487‑2778 or visit their website for help.
Please refer to the Department of State’s guidelines regarding applying for a passport for more comprehensive information.
